This form should be filled out in person at the Crux Facility to properly register and finalize payment to begin your membership.

Use this form to request membership changes such as freezes or terminations. All freeze and termination requests must be submitted a minimum of 5 days prior to billing on the 10th in order to take effect for the coming month.
